Medical Records Requests:

This practice is closed. Medical records will be securely retained in accordance with Oregon law for at least seven (7) years from the last date of care.

To request records be sent to a new provider, please email: [email protected] and include a completed Release of Information Form. Once the request is received, Lisa will verify client identity and provider information and records will be securely forwarded.
